You can get enrolled by simply filling out the "Enroll Now" information or calling 813 968-1207.
Can I keep these policies when I am no longer a federal employee?
Yes, these policies are portable at the same price.
Can I cover my spouse and children?
Yes, you can cover your spouse if enrolled in the plans prior to age 75.. Your children can be covered up to age 26 and there is no charge for them if enrolling in the cancer plan.
Are the premium rates guaranteed?
Yes, the premiums are guaranteed renewable if the premiums continue to be paid either through payroll or on a direct basis.
How are claims paid?
Claims are paid directly to employee electronically through ONE DAY PAY or submitting a paper claim form directly to Aflac with required documentation.
What about pre-existing conditions?
Please refer to specific brochures for each product for pre-existing conditions and exclusion information.
At what age to the policies terminate?
The policies will continue to be in-force as long as the premiums continue to be paid. They will not terminate due to age.
Is skin cancer covered under the cancer plan?
Yes, skin cancers are covered as described in the brochure. Please refer to the brochure for pre existing limitations and exclusions for coverage.
